Waverly, Ky – The Kentucky Office of Mine Safety and Licensing is investigating a morning accident that killed a Union County coal miner. 39-year-old James Falk of Slaughters died after a shuttle car hit him at the River View Coal Mine in Waverly. The underground mine employs 437 workers. Federal records say another miner was seriously injured at the site in September when a piece of roof rock fell on him, fracturing his back. This is the sixth coal miner death in Kentucky this year.
